SUPPORT COORDINATION

We can help you to get the most out of what is available to you.
Support Coordination (also called Coordination of Supports) means we can assist you to use your NDIS plan to the fullest. We help you to understand your budget and guide you through the ways your plan can be used to achieve your goals.
By understanding your plan, we are able help you navigate the NDIS, find and connect to supports, build capacity, and work with you and your providers to ensure your NDIS plan meets your needs.
We can help maximise your funding by obtaining quotes for services, Assistive Technology and home modifications where required, and help you to find providers if you’re not sure where to start.
Our friendly, professional service offers reassurance, guidance and provides ongoing support whilst navigating the NDIS funding and supports. This can include plan reviews, including where you disagree with an NDIS decision or there has been a change in your circumstances which requires a change in your plan. We also help with resolving any issues that may occur with service providers.
We can provide a Support Coordinator for you even if your care services are not with United Disability Care.
Our team have highly experienced Support Coordinators who specialise in psycho-social disability/mental health, SIL and SDA, physical disability, intellectual disability, acquired brain injury, stroke, sensory disability, and degenerative conditions.

Essentially our role is to help raise understanding for people with disability or their decisions makers, to be able to manage their NDIA plan themselves. The funding we receive is from the section called Capacity Building and not every client is allocated this in their NDIS plan. It normally depends on the complexity of the participants needs.
It is our role to build the persons capacity and understanding so that in future years they may not need a Supports Coordinator and they are able to manage the processes successfully themselves.
One of the key areas that we support is ensuring our clients have the right documentation ready for their NDIS plan reviews. Helping them to understand the requirements and the best way to collate the right information to help identify the correct level and areas of funding for their needs.
- YOUR SUPPORTS COORDINATOR CAN HELP YOU WITH:
- Understanding your NDIS plan and to identify the Supports that are available to you
- Determining monthly and weekly budgets aligned to your plan support categories
- Maximising your funding by obtaining quotes for services, equipment and home modifications where required
- Coordinating service agreements and service delivery with service providers
- Negotiating and liaising with government, NDIA and other agencies
- Resolving any issues that may arise with service providers
- Your annual plan review, requesting early plan review or to submit a change of circumstances to the NDIA
Support Coordination is available at 3 levels:
- Support Connection
- Coordination of Support Services
- Specialist Support Coordination

Plan Review
Your plan is reviewed annually and your Support Coordinator can work with your Plan Manager. The review is to ensure the services are meeting your needs and that the allocations for services match your budget. You can request and early plan review if you feel your current plan isn’t meeting your needs.
Sometimes the smallest of changes, can make the biggest impact in the quality of life for the people we support.

NDIS has really changed the landscape of the disability sector.
